L’insufficienza cardiaca avanzata rimane una delle patologie più frequenti e complesse nella popolazione adulta generale, e richiede pertanto un trattamento tempestivo e adeguato al fine di preservare esiti clinici accettabili. L’attuale carenza di donatori limita la possibilità di terapia sostitutiva cardiaca basata esclusivamente sul trapianto di cuore. Oggi, gli sviluppi ingegneristici di tecnologie affidabili e durature nell’ambito dei sistemi di Supporto Meccanico al Circolo e di Cuore Artificiale consentono di affrontare diversi scenari clinici e di trattare in modo appropriato sia i pazienti con insufficienza cardiaca acuta sia quelli con insufficienza cardiaca cronica. Pompe sempre più versatili, intracorporee e miniaturizzate sono ormai adottate nella pratica clinica quotidiana. Il tempestivo “know-how” di medici, tecnici e ingegneri offre una valida soluzione terapeutica nell’impiego del Supporto Meccanico al Circolo come ponte al trapianto cardiaco e/o come “destination therapy”, cioè supporto permanente alternativo al trapianto di cuore. Il ruolo dei bioingegneri è essenziale nel monitoraggio dei sistemi, nella gestione degli eventi avversi correlati al supporto meccanico al circolo e nello sviluppo futuro di dispositivi completamente impiantabili, che potrebbero rappresentare il futuro gold standard terapeutico nell’insufficienza cardiaca terminale refrattaria. L’obiettivo del corso è fornire ai partecipanti una panoramica completa degli aspetti tecnici, ingegneristici e clinici di questa terapia delicata e promettente. Cardiochirurghi, cardiologi, anestesisti e perfusionisti, riconosciuti a livello internazionale come esperti del settore, saranno invitati a tenere lezioni in presenza. È previsto un test finale volto a valutare il livello di apprendimento complessivo dei partecipanti.
Advanced heart failure remains one of the most frequent and demanding disease in the general adult population thus deserving tempestive and adequate treatment to preserve acceptable outcomes. Lack of donors currently limits the option of heart replacement therapy by the sole adoption of heart transplantation. Currently, the engineering developments of reliable and durable technologies in terms of temporary and durable Mechanical Circulatory Support/Artificial Heart systems result to address any kind of clinical scenario and treat properly both acute and chronic heart failure patients. More and more versatile, intracorporeal and miniaturized pumps have been adopting in the daily clinical practice. The timely ‘know-how’ of physicians, technicians and engineers provides a valid therapeutic solution in the adoption of Mechanical Circulatory Support as bridge to Heart Transplantation and/or ‘destination therapy’ (permanent support for alternative to Heart Transplantation). The role of bio-engineers would be essential in the monitoring of systems, management of MCS-related adverse events and further developments of fully implantable devices which might be the future gold-standard of therapy in refractory end-stage heart failure. The goal of the course would be to provide fully, to the attendees, all technical, engineering and clinical aspects of such a delicate and promising therapy. Cardiac Surgeons, Cardiologists, Anesthesiologists, Perfusionists who are addicted and internationally known as experts on the issue are invited to give, in person, lessons. A final test will be fixed and set to evaluate the level of learning of overall attendees.
