Description Abdominal wall pathology is the most frequent cause of surgery in all hospitals. The first cause is the inguinal hernia followed by the umbilical hernia and the laparocele (postoperative or incisional hernia in Anglo-Saxon terminology). In these last two areas, the closure of the hernial defect has always been the major difficulty of the intervention. Historically relying on the direct suture of the tissues, even with the reinforcement of nets, the intervention still pays a high rate of failures and relapses. Laparoscopic surgery, which is in great demand today, is even more difficult to apply due to the lack of suitable means for reconstructing the anatomical-functional integrity of the abdominal wall.

Starting from this assumption, the object of this thesis is the development of an innovative device whose main purpose is to overcome the limitations of the traditional suture, such as the identification of a modest amount of tissue included in the suture with respect to the muscle mass and to the forces exerted, and the induction of ischemia induced on the tissue margin undergoing healing.

The present device aims to approximate the tissues by moving the point of application of the forces away from the scar line so as to distribute them homogeneously along the entire abdominal wall. It consists of a pair or more series of spirals that are inserted perpendicularly to the edges of the fabric to be approximated and then joined together with a simple fastening system.

The purpose of this thesis is:

A) Identification of the optimal structure of the spiral.

B) Identification of a specific material that meets the mechanical characteristics required by the technology in question (distinguishing between absorbable and non-absorbable materials).

C) The creation of some prototypes of the device that will be applied to biological tissues and whose resistance will be evaluated by performing mechanical tests in the laboratory.

D) Design of the applicator useful for inserting the device in the surgical field.

After the experimental phase in the laboratory has been completed, the mechanical strength of the device on human cadeveric tissues is evaluated.

Required skills This thesis foresees the manipulation of biological tissues. A high level of motivation is required for scientific research and a willingness to work in a team.
