Description Design of a smartphone application for the management by caregivers (those who take care of the child, usually the parents) of patients with cardiac pathology, undergoing transplantation and/or cardiac surgery. This tool should be able to support and sustain parents at home after hospital discharge, thanks to the transmission of useful information regarding the care and wellbeing of the child. The tool should also allow caregivers to interact with the application in order to better organize the care pathway. Specifically, the management of pharmacological therapy is described by caregivers as complex and difficult, often involving many medications to be administered with precise timing and methods of preparation and intake; forgetting and/or mistakes increase the risk of developing serious complications for the child's health. The care of personal hygiene and surgical wounds through proper and correct dressing of the site is essential, and as with other topics including diet, vaccinations, sports, contraceptive methods for adolescents, etc., there are specific guidelines and special attention to follow. Regarding all this, having a 24/7 application that encompasses all the necessary information to best assist patients with cardiac pathology and the possibility to interact with it to organize and personalize care (reminders, calendars, notes, language, etc.) would help reduce therapeutic errors, complications, and health risks (and subsequent hospital readmissions) and would increase therapeutic adherence, autonomy, and caregiving skills of the parents.
